According to the report, a 2006 Ford Focus was banner westbound on the SR 18, just west of Foster Road, when it actually entered the eastbound lanes; it was struck face-to-face by a tractor-trailer.

The driver of the Focus, 16-year-old Terrence T. Haynes of Wellington, Ohio, was dead on spot.

Steven L. Watson, the driver of the tractor-trailer was sustained injuries to his right hand. Watson, 59, is from Rochester, Pennsylvania.
